Each of the following fields are required for participation in the clinic, and must be submitted by a parent or legal guardian. Your form is not submitted until you fill all required fields and press the 'Submit' button at the bottom of this page.
I hereby agree and consent to the following parameters as conditions of my child's participation in the Maui Adult Baseball League (MABL) Youth Clinic (hearinafter referred to as "Clinic") to be held on Sunday, May 22 at Iron Maehara Baseball Stadium:
Parent/Guardian acknowledges and understands that the Clinic may involve physical contact and may result in bodily injury to participants. Parent/Guardian further acknowledges and understands the risks associated with participant's participation in the Clinic and is fully aware that there may be unforeseen risks associated with participant's participation in the Clinic.
Parent/Guardian shall release, waive, discharge, and covenant not to sue the Clinic organizers or their agents from any and all losses, liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant or any property belonging to participant, whether caused by the negligence of Clinic organizers, or otherwise, during the participant's participation in the Clinic, or while in, on, or upon the area where the Clinic is being conducted.
Parent/Guardian further shall indemnify and hold harmless the Clinic organizers from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to the participant's participation in the Clinic, whether caused by the negligence of the Clinic organizers or otherwise.
Parent/Guardian confirms that the participant is covered by medical insurance that is sufficient to cover any medical expense that may be incurred due to the participant's participation in the clinic or by the participant's presence in, on, or upon the area where the Clinic is being conducted.
Parent/Guardian agrees to allow Clinic organizers to take photographs/video of the participant, along with the right to use, alter and publish the photographs. Clinic organizers may use said photographs/video for any lawful purpose with or without Parent/Guardian or participant’s further consent. Parent/Guardian further releases the Clinic organizers from all claims or liability related to said photographs/video.
